When Does Lower Back Pain Need an X-Ray?
One of the most common questions I hear from patients with lower back pain is, “Do I need an X-ray?”
The answer is: not always.
Most episodes of back pain improve with conservative treatment and do not require imaging. However, there are situations where an X-ray can provide valuable information and help guide treatment decisions.
When an X-Ray May Be Helpful
An X-ray may be appropriate when a patient has experienced a significant injury, such as a motor vehicle accident or a fall. It can also be useful when pain is severe, persistent, or accompanied by symptoms that suggest a more serious underlying condition.
Additionally, patients with a history of osteoporosis, previous fractures, cancer, or certain systemic diseases may benefit from imaging as part of their evaluation.
What an X-Ray Can Show
X-rays provide detailed information about the bones and joints of the spine. They can reveal:

Can an X-ray show a herniated disc?
No. X-rays visualize bone and joint structures but do not directly visualize discs. MRI is typically preferred when disc injury is suspected. While X-rays do not show muscles, ligaments, or discs directly, they often provide important clues that help explain a patient’s symptoms.
Why Every Patient Doesn’t Need Imaging
Many patients assume that more testing is always better. In reality, unnecessary imaging can increase costs without improving outcomes.
A thorough history and physical examination are often the most important tools in determining the cause of back pain. Imaging should be ordered when it will provide information that may influence treatment recommendations.
